Junior Elite Program - Liability Waiver and Participant Terms and Conditions *

I, the undersigned parent or guardian, certify that I have the legal authority to complete this registration and hereby release the Junior Elite Golf Program ("JEGP"), its officers, directors, employees, agents, sponsors, host facilities, and principals from any injuries, losses, or liabilities arising from or related to the participation of my child in any JEGP event. In the event of an emergency, I authorize the JEGP to seek appropriate medical attention for my child, and I acknowledge that any costs related to such treatment will be my responsibility. Furthermore, I grant permission for the JEGP to use my child's name and likeness in promotions for the program.
